Not a medical device. Not a diagnosis, dose, or listing recommendation.

What it computes

Sum the Passamonti 2010 DIPSS (dynamic international prognostic scoring system) for primary myelofibrosis and return low (0), intermediate_1 (1–2), intermediate_2 (3–4), or high (5–6).

When to use

When an agent needs the published DIPSS point total and risk group and must not invent a myelofibrosis diagnosis or treatment choice, and must not substitute DIPSS Plus, aaDIPSS, or Cervantes 2009 IPSS.

When not to use

  • Not a medical device and not a myelofibrosis diagnosis or treatment choice. magent does not examine the patient.
  • This is Passamonti 2010 DIPSS (all-ages Table 3) only, not DIPSS Plus, not aaDIPSS (Table 4), and not Cervantes 2009 IPSS-at-diagnosis.
  • magent does not read a smear or assay hemoglobin or WBC. hemoglobin_g_dl, wbc_10e9_l, and blasts_percent are caller-supplied labs.
  • Constitutional symptoms (weight loss >10% in 6 months, night sweats, or unexplained fever >37.5°C) are a single caller-assigned boolean.

Formula

DIPSS = age_gt_65 1 + wbc_gt_25 1 + hemoglobin_lt_10 2 + blasts_ge_1 1 + constitutional_symptoms 1; Passamonti 2010 Table 3; max 6; low 0, intermediate_1 1-2, intermediate_2 3-4, high 5-6

Worked example

Age 50, no adverse factors

Given: age=50blasts_percent=0constitutional_symptoms=falsehemoglobin_g_dl=12wbc_10e9_l=10

  1. age 50 is not >65 → 0; hemoglobin 12 ≥10 → 0; WBC 10 ≤25 → 0; blasts 0% <1 → 0; constitutional_symptoms false → 0
  2. Score = 0 (max 6); risk_group = low

All five adverse factors

Given: age=66blasts_percent=1constitutional_symptoms=truehemoglobin_g_dl=9.9wbc_10e9_l=25.1

  1. age >65 → 1; hemoglobin <10 → 2; WBC >25 → 1; blasts ≥1 → 1; constitutional_symptoms → 1
  2. Score = 6 (max 6); risk_group = high (5–6)

Parameters

Name Type Required Description
age integer required Age in years (18-120). Age >65 scores 1 (Passamonti 2010 Table 3). Age 65 is the ≤65 arm and scores 0.
hemoglobin_g_dl number required Hemoglobin in g/dL (3-20). <10 scores 2 (Passamonti 2010 Table 3; anemia HR 4.18). 10.0 is the ≥10 arm and scores 0. magent does not assay hemoglobin.
wbc_10e9_l number required White-cell count in 10^9/L (0.1-500). >25 scores 1 (Passamonti 2010 Table 3). 25.0 is the ≤25 arm and scores 0. magent does not assay WBC.
blasts_percent number required Peripheral-blood blasts as a percent (0-30). ≥1 scores 1 (Passamonti 2010 Table 3). 0.9 is the <1 arm and scores 0. magent does not read a smear.
constitutional_symptoms boolean required Weight loss >10% in 6 months, night sweats, or unexplained fever >37.5°C as assigned by the caller (Passamonti 2010 methods). true scores 1. magent does not take a history.

Bulk (POST)

POST the same path with a JSON items array. The first item is the GET unit; each additional item is $0.002. Invalid items return HTTP 400 before settlement.

POST /api/calc/dipss Max 25 unit_amount + extra_item_amount * (n - 1) extra $0.002 PREVIEW

  • 1 to 25 items. Each item uses the same keys as the GET query parameters.
  • POST of 1 item costs the same as GET. Lists longer than 25: chunk into multiple POSTs. Extra items still execute; do not pad dummy rows.
  • The first invalid item fails the whole request (index in the 400 JSON). You are not charged.
  • HTTP 200 is {count, path, items}. Each items[i] is the GET 200 body for that row.
  • Catalog misses (empty matches, found=false) stay paid 200s inside that slot.
  • x402 resource URL is this path plus n and body_sha256. Browsers that prefer text/html get the same paywall as GET; Pay retries POST with the same items.

Related tools

  • IPSS-R — Sum the Greenberg 2012 IPSS-R (revised international prognostic scoring system) for myelodysplastic syndromes and return very_low (≤1.5), low (>1.5–3), intermediate (>3–4.5), high (>4.5–6), or very_high (>6).

Payment

Unpaid requests return HTTP 402 even if query parameters or POST items are missing or invalid, so CDP and agents can index the path. After a PAYMENT-SIGNATURE, invalid params return HTTP 400 JSON before settlement. magent does not charge. Fix the params, then request a new 402 quote for that complete URL. Catalog misses (empty matches, found=false) are valid paid 200s. Unpaid valid requests return HTTP 402 with a base64 PAYMENT-REQUIRED header. HTTP 503 is not a new quote: retry the same PAYMENT-SIGNATURE. Full handshake and shared payment errors: Payments. Stdio MCP: /docs/mcp.

Guarantees

  • HTTP 402 is unpaid, including a missing query. After PAYMENT-SIGNATURE, invalid parameters return HTTP 400 before settlement. You are not charged. HTTP 402 never includes the tool JSON.
  • exact: settlement finishes before the tool (paymentFlow upfront). batch-settlement: verify/deposit before the tool; chargedCumulativeAmount commits after HTTP 2xx. Uncertain or failed settlement fails closed (HTTP 503). Retry the same PAYMENT-SIGNATURE.
  • HTTP 402 is unpaid only. Do not sign a second authorization because of a 503.
  • Execution is timeout-bounded and concurrency-capped.
  • Calculator inputs are stored encrypted for allowlisted operators (Magent Console). They are not in public logs, SSH lookups, catalog, or rollups. Request logs store path without query.
